Mooring Rental Registration, Payment and Documentaion
By signing up through this mechanism you will be able to pay for your seasonal Mooring, read and agree to our contract and provide your Insurance documentation. If you have any questions about how complete registering for your mooring, call OUW during their open hours at 608-262-1630.
If you would prefer to provide this documentation in person at OUW you are able to do so.
Instructions:
1. Select your mooring field payment Option (Field #1, Field #2 or Field #4)
2. Input your payment information
3. Read and Agree to the contract by clicking on the "I accept these terms" box
4. Answer in-full the Insurance Questionaire
5. Answer the boat information questionaire - Event Type
- Accommodation Information
- Cancellation, Reschedule, and Refund Information
- Driving or Travel Details
- Equipment/ Gear Required by Participant
- Food and Equipment Information
- Safety plan information to be shared with attendees:
